Betty C. Riddell Award for Pharmacy Practitioner Excellence

This award will be presented annually to a member of the Branch in recognition of outstanding achievement to patient care, contribution to the profession and long term service to the practice of pharmacy. Selection of nominess will be made on the basis of involvement and significant contribution to the practice of pharmacy at the pharmacist's work place. Please take a moment to write the Awards committee in order to recognize the outstanding accomplishments of a fellow Society member.

The Saskatchewan Branch CSHP Merit Award

The award is presented annually to a practising hospital pharmacist and member of the Branch in recognition of a project which could easily be accomplished regardless of hospital size or staff, which need not be sophisticated, and which serves a useful purpose or has recently been published.  We know that many pharmacists have initiated exciting programs in their hospitals and we would like to recognize them.  Take a few minutes to write the Awards Committee about when your project started, how it was implemented, and how it is running, or let us know about a recent publication.

This award is presented annually to a member of the Branch in recognition of promising leadership, dedication and commitment to practice excellence and professional growth. Selection of nominees will be made on the basis of contribution to patient care, to research or education, and service to the profession. Candidates must have no more than 5 years of clinical experience to be eligible. Take a moment to recognise the efforts of our newest colleagues by nominating them for this award.

This award is given annually to a deserving Saskatchewan hospital pharmacy resident in recognition of a residency project which represents a significant innovation, practical application and/or development in an institutional pharmacy practice setting. If you are currently a hospital pharmacy resident or have completed such a program in the previous year, please send in your project manuscript for consideration of this award.

The Pharmaceutical Care Award

(This award is no longer being presented)
This award was presented annually to three practising hospital pharmacists and members of Saskatchewan Branch CSHP in recognition of outstanding achievement in pharmaceutical care.  Hospital pharmacy residents and members with advanced degrees (M.Sc, Pharm.D., Ph.D.) were not be eligible for this award.  The award was presented based on the description of a pharmaceutical care interaction that displayed the promotion of positive patient outcomes.

BETTY C. RIDDELL AWARD FOR PHARMACY PRACTIONER EXCELLENCE

TERMS OF REFERENCE

  1. DESCRIPTION
    This award consists of an engraved plaque and cash prize of $500 given annually to a member of the Saskatchewan Branch of The Canadian Society of Hospital Pharmacists in recognition of outstanding achievement to patient care, contribution to the profession, and long term service to the practice of pharmacy. Selection of nominees will be made on the basis of involvement and significant contribution to the practice of pharmacy at the pharmacist's work place.
     
  2. CALL FOR NOMINATIONS
    The Awards Committee of the Saskatchewan Branch CSHP will call for nominations for the "Betty C. Riddell Award for Pharmacy Practioner Excellence" on a yearly basis as part of the branch awards process.
     
  3. SUBMISSION OF NOMINATIONS
    The nomination of a candidate for this award shall be submitted no later than September 1st of each year. The successful nominee will be presented with their award at the Saskatchewan Branch Annual General Meeting in October of the same year.
    All nominations shall be submitted to the Awards Committee, in writing, with substantiating background information. Submissions may be sent electronically to the Awards Committee Chair.
    All nominees shall be reviewed against the criteria for the award by the Executive Committee of the Saskatchewan Branch CSHP.
     
  4. SELECTION OF CANDIDATES
    The candidate with be a member in good standing of the Saskatchewan Branch CHSP and selection shall be based upon:
    1. Long term service to the practice of pharmacy (minimum of 10 years).
    2. Excellence in the provision of patient-focused care.
    3. Committment to excellence by involvement in initiatives aimed at improving pharmacy services within the workplace.
    4. Strong role model for students, residents and other pharmacists.
    5. Innovative practitioner with a sound work ethic.
    6. Ambassador of the profession.
    7. Other qualifications, related experience and professional development may be considered during the selection process.

THE OREST BUCHKO HOSPITAL PHARMACY AWARD

TERMS OF REFERENCE

  1. DESCRIPTION
    This award is given annually to a graduating student of the Bachelor of Science in Pharmacy Program at the University of Saskatchewan pursuing a career in Hospital Pharmacy. The award will be presented to the successful applicant at the Convocation Luncheon. The recipient will receive a cash prize on $500 and a certificate at the SK Branch CSHP AGM in October.
     
  2. SUBMISSION OF APPLICATIONS
    The application for this award shall be submitted to the College of Pharmacy and Nutrition not later than March 31.

    An application consists of: (1) letter of application (2) current CV and (3) a brief description of the student's interests in the field of hospital pharmacy practice and career plans in this profession. All applications shall be reviewed by a College of Pharmacy and Nutrition Scholarships and Awards Committee and a member of the Executive of the Saskatchewan Branch CSHP.
     
  3. SELECTION OF CANDIDATES
    The candidate with be a student member in good standing of the Saskatchewan Branch CHSP. Selection shall be based upon:
    1. Performance in Structural Practical Experience Course (Pharmacy 480.4)
    2. Academics: general overall average with specific emphasis on Hospital Pharmacy courses such as Hospital Pharmacy Practice (Pharmacy 462.3), Pharmacotherapeutics (Pharmacy 455.7, 456.7, 557.6), Patient Care II & III (Pharmacy 465.2, 565.2), Evidence Based Practice (Pharmacy 472.2), Pharmacokinetics and Biotherapeutics (Pharmacy 307.2)
    3. Involvement in the Saskatchewan Pharmacy Students' Society Council, CAPSI or other professional or volunteer groups.

THE "PHARMACIST OF THE YEAR" AWARD

TERMS OF REFERENCE

  1. DESCRIPTION
    This award consists of a $500 prize and an appropriately engraved individual plaque given annually to a member of the society in recognition of outstanding achievement and long term service to the practice of hospital pharmacy. Selection of nominees will be made on the basis of involvement and significant contribution to hospital pharmacy at the local, provincial or national level.
     
  2. CALL FOR NOMINATIONS
    The Awards Committee of the Saskatchewan Branch CSHP is seeking nominations for the "Pharmacist of the Year"Award.

THE J.L. SUMMERS ACHIEVEMENT AWARD

TERMS OF REFERENCE

  1. DESCRIPTION
    This award consists of an appropriately engraved individual plaque and a cash award of $1,000.00 given annually to a member of the society in recognition of a significant contribution within the practice of hospital pharmacy which demonstrates innovation and leadership in the health care field.
    The cash award may be used to attend a major conference pertaining to the individual's area of interest in the practice of pharmacy or other pharmacy related activities.
     
  2. CALL FOR NOMINATIONS
    The Awards Committee of the Saskatchewan Branch of CSHP is seeking nominations for the "J.L. Summers Achievement Award".
     
  3. SUBMISSION OF NOMINATIONS

    This award consists of a certificate and a cash award of $500.00 given to a practicing hospital pharmacist(s) and member(s) of the society in recognition of a project which could easily be accomplished regardless of hospital size or staff, which need not be sophisticated, and which serves a useful purpose or has been recently published.
     
  2. CALL FOR APPLICATIONS
    The Awards Committee of the Saskatchewan Branch CSHP is seeking applications for The Saskatchewan Branch CSHP Merit Award.
     
  3. SUBMISSION OF APPLICATIONS
    The nomination of a candidate for this award shall be submitted no later than September 1st. The successful nominee will be presented with their award at the Saskatchewan Branch Annual Meeting in October of the same year.
    All nominations shall be submitted to the Awards Committee in writing, with substantiating background information. Submissions may be made electronically to the Awards Committee Chair. The Awards Committee shall blind all submissions (i.e., remove all identifying names, locations, etc.) before distribution to judges.
    All nominees shall be judged by the Merit Award Assessment Committee.

    The candidate will be a member of the Saskatchewan Branch CSHP and selection shall be based upon:
    1. A project serving a useful purpose, community service project or a recent publication.

THE NEW HOSPITAL PHARMACY PRACTITIONER AWARD

TERMS OF REFERENCE

  1. DESCRIPTION
    This award recognizes a hospital pharmacy practitioner who through their service to patient care, to education or research, to the profession and the Society, is worthy of a recognition that denotes promising leadership, dedication and commitment to practice excellence and professional growth. To be eligible, candidates must have no more than 5 years of clinical experience. Selection of nominees will be made on the basis of contribution to hospital pharmacy, in the provision of patient care and service to education, research and society involvement. The winner will receive a $1000 cash award and an engraved plaque that will be presented at the SK Branch AGM in October.
     
  2. CALL FOR NOMINATIONS
    The Awards Committee of the Saskatchewan Branch CSHP is seeking nominations for the “New Hospital Pharmacy Practitioner” Award.
     
  3. SUBMISSION OF NOMINATIONS
    The nomination of a candidate for this award shall be submitted to the Awards Committee in writing (electronic permitted) with substantiating background information and endorsed by an additional CSHP member.
     
  4. SELECTION OF CANDIDATES
    All nominees shall be reviewed by the Executive Committee of the Saskatchewan Branch of CSHP. The candidate will be a member in good standing with the Branch and selection shall be based upon:
    1. No more than 5 years of involvement in hospital pharmacy practice

THE HOSPITAL PHARMACY RESIDENCY AWARD

TERMS OF REFERENCE

  1. DESCRIPTION
    This award recognizes hospital pharmacy residents whose major projects are judged to be most deserving in terms of significant innovation, practical application, and/or development in an institutional pharmacy practice setting. To apply for this award, each resident must provide 4 copies of their residency project manuscript which has been made anonymous (ie. all identifying names and locations should be removed). The manuscript format must follow the Canadian Journal of Hospital Pharmacy criteria.

  3. CRITERIA
    To be eligible, each candidate must be:
    1. Enrolled currently in a CHPRB-accredited hospital pharmacy residency program or have completed such a program in the previous year
    2. member of the Canadian Society of Hospital Pharmacists (as of October 1st of the year of their residency program).

  4. AWARD
    The Hospital Pharmacy Residency Award will consist of a cheque for $1,000.00 and an engraved memento. Each branch will be entitled to award 1 Hospital Pharmacy Residency Award for each 10 pharmacy residents studying in the province who are CSHP members. The number of awards that can be distributed by each branch is as follows:
    • New Brunswick and Nova Scotia - 1 award
    • Quebec - 5 awards
    • Ontario - 2 awards
    • Manitoba - 1 award
    • Saskatchewan - 1 award
    • Alberta - 1 award
    • British Columbia - 1 award

    Procedure:
    1. All CSHP Branches will advertise and administer the Hospital Pharmacy Residency Award program annually within their respective Branches.
    2. Adjudication of submissions for the Award will be conducted by 3 selected CSHP SK Branch members (preferable, the reviewers would include 2 college faculty and one rural pharmacist to prevent bias in the judging process) blinded to the author and location of the project. Adjudicators will be chosen by each Branch administering the Award. An evaluation form will be provided to adjudicators by the respective Branch.
    3. Presentation of the Hospital Pharmacy Residency Award to the winner will be conducted at the SK Branch Annual General Meeting in October. Local Pfizer Canada representatives are to be invited to participate in the Branch presentation of the Award.
    4. Pfizer Canada's support of this national award program will be recognized by the Branches and CSHP National. All Branches will acknowledge the Hospital Pharmacy Residency Award recipients in their respective newsletters. CSHP National will recognize the Award recipients in the Canadian Journal of Hospital Pharmacy (CJHP) and at the annual Awards Luncheon at the Professional Practice Conference.
    5. By November 1 of each year, all Branches administering the Hospital Pharmacy Residency Award will forward the following to CSHP National: Name(s) of their Hospital Pharmacy Residency Award recipient(s) & Abstract(s) of the winning residency project(s)
    6. Upon receipt of the funds from Pfizer Canada, the CSHP National office will distribute the funds to the branches for the issuing of cheques by the branches to the winners. The CSHP National office will obtain the engraved mementos and forward these to the recipient of each award or to a representative of the branch, according to the wishes of each branch.
    7. The CSHP National office will invoice Pfizer Canada Inc. each year for the total cost of this award program, which includes the monetary value of the awards, the cost of the engraved mementos, and a charge for administration of the awards.
